Senior Project Reflection

Part 1:
(1)   Two hour presentation student assessment
I feel I deserve an AE on my two hour presentation because I met all the requirements on the rubric and I feel I had great audience participation. The students in  my presentation were intrigued and learned a lot, the activities were great, I felt many students did play the role well. The roles of a midwife, mother and a mother in labor. I may have been a little nervous at first but everyone is, and after a minute I shook it off and was able to inform the class while not making it boring. So in the end the students did learn a lot, which was my goal. They  really grasped the concept and I feel that they walked out knowing so much more on both sides of maternity care.

(2)   What are you most proud of in your 2 hour and why?
I am most proud of the audience participation, I liked answering all their questions. They asked me questions after I was done presenting even during break. I felt that them asking questions showed their interest in my topic, and I know the fact that I stood their confidently and passionately about my topic allowed for them to become interested in midwifery. The students participation made me very happy and proud. Midwifery is not a popular or a familiar topic, so I thought engaging them would be difficult but it wasn't and I was very pleased with my audience:)

Part 2:
(1)   What do you think you did well on in the project? What could you have done better? Please explain.

I think everything went well in this project, my research was great, my I-search was good, and my independent components were good too. I especially enjoyed my presentation, I realized that I LOVE talking about midwifery. My favorite parts of the projects were presenting of the project like the 20 min presentation. Preparing for it may make me nervous but it allows for me to see all the quality work I have put into this project. Reading all my books, and driving all the way to Chino every week paid off! What could have gone better would have to be my science fair project, I really struggled on that I was not able to produce a quality project. I honestly think it took up my time, and feel that should be taken out for next year.

(2)   What is your EQ and what is the best answer to your EQ?
EQ: What is the best way a midwife can influence a birth experience?
Best Answer: Educating the mother

(3)   How has the last month of culminating events (e.g I-Search, Two Hour, Exit etc) affected your answer to your EQ? Has it changed? Why or why not?
This last month did change my answer, when we were practicing for exit interviews and I was asked what is your best answer and why? I realized there that supporting the natural process was a great answer but not the best and I would get stuck with that being my answer to the question. At first I didn't even have educating the mother as an answer but after reviewing my interviews, research and talking to Mrs. Ortega I came to the conclusion that educating the mother was a key element in my EQ, and it allowed for me to connect all my answers.

(4)   What suggestion do you have in order to improve the senior project?
People need to work on their research, the quality of research will play a big role in the final presentation, so making sure they do good research and actually margin note it is important. I think that for next year the teachers should make it mandatory to check for 30,50,60 sources and grade it.

(5)   Overall Senior Project Assessment
P+ -- AE
I feel that I did accomplish a lot in the project, every component was good, and my end product was great:)
Also my 2hr was really good too!

(1) LIA Response to blog:
Literal

My usual days were Mondays and Wednesdays, Mondays were office days so there were no appointments. Those days I would do research and usually talk to Lynn, SM and administrator director or Joyce CPM. Talking to them about my research or there baby delivery stories, and about my essential question helped me so much. When they did have appointments I would talk to the parents about why they chose a midwife. I learned a lot about the people that went in to the birth center. Many parents had children so while they were in there prenatal or post natal check ups I would entertain their kids. My midwife always had games or videos out there for them. Some days there were students that would come to the birth center, and the students were always fun! Joyce would do tours for them and explained the midwifery model of care, and many students seemed as they were interested about midwifery, like me! Service learning has allowed for me to be apart of a hypnobabies class and a child birthing class, those classes I learned why parents have chosen midwives and how well educated they all are. So basically, I attended one prental, not a lot I know but I still learned a lot, I talked to the parents, the kids, the students that came in and joined two classes.

Joyce Thomas: 909-591-2924 cell: 909-489-7077

Interpretive
The most important thing I gained was my product. I saw how Lynn and Joyce ran their office and sometimes had problems with being paid. Midwives sometimes struggle with finances due to the fact that health insurance will not pay them, and sometimes families cannot pay them. Experiencing this I verified that I do not want my own private practice. Not because of the getting paid issue but because I cannot picture myself fighting over money, and I would be to stressed about running my own business. I may not even want to be a CNM in California, I want to practice in a hospital in another state, and mainly help those in rural areas. So I will already know that they have financial problems. I know for a fact that I do want to be a nurse midwife, my fourth interview guided me in the right direction, and I met her through service learning. Another thing I gained was my relationship with my midwife, I can talk to her all day, and although I am done with my hours I will always go back to visit her. I am very grateful for having her in my life because with out her I would not have learned as much as I learned, she was very welcoming. Anytime someone new came in she introduced me and I felt she gave me my place. I can most definitely go back to a student midwife with her, like she once told me.

Applied

Being at service learning influenced my answers. I saw how Joyce and Lynn educated every mother and her partner as they walked in. During a tour with new clients they would read the contract and watch the video "Birthday" and that educated them the midwifery aspect on birth. Joyce shows that video frequently. Being at service learning I saw the value of education, and why mothers chose a midwife. Many that I talked to did not have a good experience in the hospital with their previous child, others were very opened minded and knew a little about midwives. Joyce gave out her "Water Birth Guide" to mothers, and also told them about a hypnobabies class they could take. I knew that Joyce was educating them on their options so they could have a better birth experience.
I also saw the bond they built, Joyce and Lynn could talk to their clients about their day, and never spent 15 min with them it was always about 45 min.
learning about the parents lives and their previous births allowed for me to understand midwifery and why they chose to deliver with a midwife.
My midwife also educated them on the natural process and on how important it is to follow it. Joyce showed them techniques for dealing with the pain and explained to them what the hospital would do (This was so helpful to me) she also told them that they only take low risk patients, and the hospital is for high risk patients. I didn't understand this but when a mother came in and she didn't have any prenatal check ups, no medical history available, then she became too high risk for them. Service learning allowed for me to see my answers in real life, and was later supported through text that I read.

2 hr lesson plan

LESSON PLAN:

Sponge activity:  I plan on asking them if they have heard of a midwife, and what they do. Explain the term and how it means "with woman" and how they really are with woman the whole time. i will also ask if they have heard of water birth and what the water does. Many people think that the baby can drown in the water, so i will elaborate on how the baby cannot drown. So for the sponge i will simply ask questions pertaining to what i will be discussing.

Introduction:  What is the best way a midwife can influence a birth experience?
Words:
midwife: A female in most cases who delivers babies, they tend to intervene less and empower the mother by helping her deliver her baby naturally.
Influence: The midwife must educate the mother, build a bond and support the natural process in order to influence the mother’s birth experience. She has a big role in her pregnancy, labor and birth so whatever she says or does will be obvious in her birth. She has the power to help the mother and assist her while she has a beautiful birth.
Birth experience: (I know two words but they go together) The whole experience can be very empowering to woman, and society associates birth with pain. So if the mother can over come it with no drugs then she has come out as a fighter. Birth experiences are physical
and psychological. A birth experience only happens once but that experience and memories can go for the rest of a family's life.
I chose this topic because it is the career path that I want to be in. My grandma was a midwife and the summer before I went into senior year she told me her baby delivering stories and that encouraged me.
Plan for the day: After the sponge activity, I will talk about my topic why I chose it, explain my EQ, state the first answer and then do the activity.
Explain my 1st answer. Then do the rest of the activity.
I will then explain my 2nd answer and do the activity.
Then my 3rd answer and how it relates to my eq.
I will give details on how they all relate to my eq at the end.
I will then conclude with what my best answer was and why.
Then my 3 meaningful sources.
I will then say my product and state my goal of the presentation, which is for students to have a better idea of what a birth experience can be.

Foundation:  I will talk about my grandmother being a midwife back in her time and how she inspired me. I will say how I want to be a CNM (certified nurse midwife) and help those in rural areas one day.

Body:

Answer 1: educating the mother
-----Activity 1:
  •  I will assign two people as partners randomly, senior with a junior.
  • Then I will ask for one to be the "expectant mother"
  • I will say okay so the mother has a pain in her lower back what should she do? What should her partner do? What should the midwife suggest for her to do?
  • For each question I will give them time to discuss and think about their answers.
  • Next question is if the labor is taking longer than expected what should the midwife suggest for the mother to do?
  • The last question is if the mother is in a lot of pain what should she do?
So the students are not educated they are not going to be able to know what to do, because they do not have enough knowledge. So by this I show that if you're not educated you cannot choose what is best for you. Then I will discuss my first answer, educate them. Ask them again and hope they answer better. I will also ask an additional question:
  • If the mother is having a difficult birth, what should she do? Difficult being she is having a hard time pushing, the baby is not coming out.
In total it will be 4 questions. Each question will be given 2 -3 min. I will ask for some to share their answers and why they chose that. At the end I may suggest something or agree with them.

Answer2: Building a bond
-----Activity 2:  So I will pair them up with other partners, junior with a senior.
·         I will give each one two papers one the midwife role the other the mother.
·         On the paper it will have three visits:
- Interview
- Prenatal #1
- Prenatal #2.
·         I will write a couple questions for each visit. I will also write things that the midwife needs to ask as well as the mother.
·         They are going to be role-playing and will have to improvise with the information that I give them.
·         Interview q’s:
-Why did you choose a midwife?
Possible answer: She may want something different, she may be unhappy with her last birth.
-Who will be at the birth?
Possible answer: Entire family, immediate family, just her partner.
Try to understand her partner’s role.
· Ask medical history: previous c-sections, history of abortions.
History of abortions may lead to negative feelings about labor. ( heart and hands 14)

Mother:
· How much experience do you have?
Possible answers: Worked in a hospital, I have my own private practice, I do home births. I have delivered breech babies, VBAC.
·Try to express your hopes and desires from the birth.
-         Will you want your partner to catch your baby?
-         Will you allow routine check ups from time to time?
-         Water birth, home birth?

Prenatal #1 qs:
· Hi how are you today?
· What have you ate today?
o        How are you feeling?
Possible answers: She may be feeling tired, fat, sad, happy or excited.
o        Do you have any fears?
Possible answers: Scared of stretch marks, scared that she will feel a lot of pain, worried that she will have a difficult labor. That her birth may not be what she is hoping for.
o        How is your partner?
o        If she is late, you may want to ask why she is late and if everything is okay?
o        Find out about her life style, like work, her daily routine?
Midwifery care is personalized care and you have to be able to build a connection.

Prenatal #2 q’s:
o        These Prenatal visits are to check the health of the mother, the baby and the emotional state of the mother. These visits are meant to provide nutritional counseling, emotional support and to build a bond. You need to make her feel comfortable with you.
o        I will give them room to ask questions, I want them to make up their own questions to see how an actual visit is. The mother is choosing the midwife to be a part of a very intimate part of her life. The students will see how hard it is to try to make a connection with someone you do not know. They need to be able to make each other comfortable not just the mother needs to feel the comfort.

o        The goal is to gain her trust, her confidence and comfort.

**At the end I will ask for them to share something intimate with their partner, if they feel comfortable enough. Or rate their comfort, discuss how they felt. They could talk about how many children they may want or how they wish to deliver their baby.

** These activities will work better if I pair them up girl and boy, so that is what I’m aiming for.

Conclusion
What is your best answer and why?

Educating the mother, because that is the best way for a midwife to influence a birth experience. The mother needs to be educated on the protocols and needs to be able to make the best decisions for her and her baby. Education is key and allows for me to connect all the answers.

What is your 3 most important sources and why?

-    “The Thinking Woman’s Guide to a Better Birth”- great book with facts that are easy to understand. It compares the hospital and midwives really well.
-    My service learning- I asked questions whenever I felt I didn’t understand something, they helped me comprehend everything. I got a lot of great information from them, and absorbed everything like a sponge.
-    My fourth interview with Maryam the CNM. - My last interview summed everything up for me. It ended all my senior project interviews nicely. Although I was upset finding out that nurse midwives and Ob’s are the same in the hospital, it was a great way to see the other side of the midwifery model of care. That was a more medical model and I understood her very well. I was able to picture myself working as a nurse midwife one day, and for the first time it felt real.

What is your product and why?

My relationship with my midwife, the fact that I feel I can go back to be a student midwife there makes me feel proud. My relationship with her means a lot to me, I feel she has opened many doors for me and when I do begin my career I have a foot in.

Independent Study Component 2

1. Log

2. Posted Evidence

Pictures of my midwife and I, and a picture of a mother that delivered her baby by hypnosis.

Happy 3. Complete the LIA:

Literal

(a) Statement saying: “I, Valeria Iniesta, affirm that I completed my independent component which represents 30 hours of work.”

(b) Explanation of what you completed.
For my Independent task I did service learning. I did 35 hours by observing, engaging and asking questions when possible.  I would go into service learning mainly on Mondays and Wednesdays, from when they would get back from lunch to when they closed.  Each time I went I would try to ask questions about my research or engaged with the parents that went to the birth center.
Interpretive

Defend your work and explain how the significant parts of your component and how it demonstrates 30 hours of work.

I enjoyed talking to the mothers that held their baby in their hands that were once pregnant. I was captivated to see the transformation they all had, the mothers were very joyful and grateful to have had a beautiful birth. I also liked talking to the family and asking why they chose a midwife, or entertaining their children while the mothers had a prenatal. Taking the time to be at service learning and engaging with everyone there represents my hours of work.  I also spoke several times to midwife whenever I had questions.

Applied:
Talking to the mothers mainly helped me answer my EQ, as well as seeing them in the before and after light. It is a different atmosphere that goes around in the birth center and they all experienced natural childbirth. I can see the way the midwife talks to them, how they have a close relationship, and how they are well educated about birth. Being around mothers who support midwifery and their principals guided me to my EQ answers. Building a strong bond (answer#1) was always done, and done through 45min to an hour pre-natal. I saw the influence the strong bond had, as well as the influence the midwife had. Their relationships still go on, and always will because the midwife was there through such an important experience. My midwife and I talked about the hospital protocols and how the midwifery model differs, she compared them for me and I saw how important supporting the natural process is (answer#2).

Creative Expression idea

I was thinking of either writing a poem to show how beautiful it is to deliver with a midwife and the smiles the mothers have through out their experience. Also to show the role of the midwife, the mom and the partner.

Or to make a collage of baby pics delivered by midwives, like some black and white and some pictures of pregnant women.

Fourth Interview Question Approval

These questions are intended for a CNM. I feel asking her will help me a lot more because this is the specific career that I want. I've interviewed a CPM and a SM, but talking to a CNM will give me the chance of actually seeing how it is.
  1. What is the best way a midwife can influence a birth experience?
  2. Do you feel a competitive environment working in a hospital with OB's?
  3. Working in a hospital as a CNM have you noticed different birthing methods, or different techniques?
  4. * Do you feel it is more beneficial delivering in a hospital or in a birth center with a midwife?
  5.  How different is the atmosphere from the hospital to a birth center?
  6. How do you encourage partner involvement while you meet with the mother?
  7. During the prenatal visits in the hospital how do you make sure that they are not like OB visits that last a quick 10-15 min visit?
  8. Out of having a private practice, and working in a hospital where do you think a CNM is more versatile?
  9. If the OB suggests using Pitocin for a "slow labor" what would your response be?
  10. Do you agree with hospital's protocol? Is it different from yours?
  11. Why do you think C-section in hospital is more popular now?
  12. Do you think C-section is overused?
  13. If the mother is afraid of delivering through C-section what words of encouragement would you give?
  14. When fetal distress is picked up would you immediately pursue c-section?
  15. What is your opinion on the drugs that the hospital uses?
  16. Why do you think that the U.S has a high infant mortality rate?
  17. What’s your opinion U.S's maternal mortality rate? (Most are due from c-section or the drugs used)
  18. How would you handle a mother who is having twins?
  19. What is the first thing you would do for a breech birth?
  20. *After the mother delivers do you give the baby right away to her?
  21. *After the delivery how often do you check up with the mother and her baby?
  22. *How do you allow the mother to bond with the baby after birth? ( hospital's may want to take the baby right away to the nursery)
The ones with a star are the ones I’m not sure about.

    Independent Study 2 Approval

    1)
I'm a little iffy on this idea, but I was thinking of taking birthing classes! Like Hypnobabies, even nutritional classes, or just first time parents classes, baby classes in general.

    2)
This will give me the opportunity to see all types of birthing methods, and to see how and why parents chose they way they wanted to birth their baby! I could get a chance to see the whole baby experience. It relates to my EQ because usually the midwife encourages the parents to take classes or to gain more knowledge on pregnancy, and on nutrition, and at that moment she has influenced her birth experience in so many ways!

    Independent study1

     Evidence!!:)

    1.Log

I have been watching videos from the start, but I began keeping a log and taking notes of all of them, starting January 7th- Feb10th.
    1/7,1/15,1/17,1/19,1/24,1/26,2/01,2/03,2/05,2/09,2/10

I mainly watched videos at service learning, and at home. I would watch numerous videos through out service learning, it was never just one video. Each video was either 30 min-1hr. The longest video I watched was 2hours. So in total 20 videos, plus the timing of notes, pausing the video, asking the midwives questions, and reviewing the notes again at home.
    3. Complete the LIA:
    Literal

    (a) “I, Valeria Iniesta, affirm that I completed my independent component which represents 30 hours of work.”

    (b) I watched a lot of birth videos! I saw many babies being born, and all the births were honestly very beautiful, I recall smiling through out the videos because the babies were so cute! Plus what the video meant and signified made me happy too!  I watched births in hospitals, I watched births at home, and watched births in water. (which could be at home too, or in a birth center) I saw all the birthing methods, and while I watched the videos I took notes, paused the video, asked questions to the midwives and jotted everything down.  At the end of every video I would summarize the video then compare it with hospital births.

    Interpretive

The significant parts of my independent comp is the thorough notes I took, and at the end comparing both birth experiences, which is part of my goal for my senior project. Comparing both experiences so people know how different these births are and the benefits of midwifery births.  This demonstrates 30 hours of work, because half of the journal is full, its not just the 20 videos I watched, its the time I took to understand everything, write it down and review it all.

    Applied

This most definitely helped me a lot answer my EQ! The independent comp served its purpose! I saw how nurturing the midwives were towards the mother, how well the baby reacted to the birth, and how much the midwife actually influenced the birth experience. The midwives supported the natural process, built a strong bond with the mother, and encouraged partner involvement. My independent component compared hospitals, birth centers, OB's and midwives.  These comparisons played a significant role in my EQ. These videos allowed me to see my EQ in a different perspective and that's how I was able to come up with my EQ answers.

    Presentation 2 Rough Draft:

Essential Question: What is the most effective way a midwife can influence a birth experience?

    (A) Introduction
I want to present on the different types of birthing methods, like water birth, and hypnobabies.  I will go over what water birth is and what it exactly does, and what hypnobabies is and how effective it is.

    2.
I want to let the class know and understand what these methods do for the woman and for her baby. I want them to realize that going to the hospital is not the only option and these experiences are not painful, BUT they're not exactly pain free either (depending on the experience)

    (B) Body

    1. Major point 1:
First I will state what different options expectant mothers have and what these options can provide for them. The options being water birth and hypnobabies. I will explain what the hospital does, to show how different these methods are. I will also state that water birth is not exactly accepted in hospitals and how water birth used to be in hospitals and is in some (very few) now.
    (3 min)

    2. Major point 2:
I will explain water birth and how great it is! I really like water birth, it's something that I wish to advocate in hospitals one day:) I feel that it's a very peaceful transition for the baby and a beautiful experience for both partners. Water birth provides women with shorter labors. The warm water also provides therapeutic benefits, I will elaborate on all of this.
" Your complete guide to water birth"
    (5 min)

    3. Major point 3:
I will explain hypnobabies and how it's full of relaxing excerises.  It's a full child education class and is before the due date, it reprograms the mind to think positive and to surround your mind, body and soul of only positive people. It's also about relaxing music you can listen to before birth and during, especially during! This class also provides scripts to read during birth, but these scripts and this special music are not listened to until that day. The other scripts and music is to prep for birth in a way, and is listened to.
    (5 min)
    -Hypnobabies class.

    4. Activity:
I'm still a little iffy on the activity. But I was thinking of pairing up two people, and having them read the hynobabies script, while playing the music that it offers. I can dim the lights and have the class be really quiet and everyone whisper it so everyone is having there on secluded time. OR maybe I can just read it to them??

    5. They can get a feel of what it says and how relaxing it is (if its done properly) the students can get hands on experience of what it does, and how it's done.
    (5-7 min)
    -hypnobabies class.
    (C) Conclusion

    1. Essential points:
-I’ll state how great these ways are, and how more people need to be informed so they can one day experience this and enjoy their labor.
-Hypnobabies and water birth is something that should not be overlooked and one day be accepted in the hospitals.
-This is connected to my EQ because midwifery offers many options that hospitals don't. Midwives care about the expectant mother, and want her to be comfortable and free enough to do what she wants and these methods offer that. If the midwife allows the expectant mother to experience her birth the way she wants, then she has most definitely influenced her birth experience.

    2. What was your most important source for this presentation and why?
    - The class I took of hypnobabies
-My Water birth guidebook for Aqua Natal
    - The many videos I have seen!

    (D) Materials needed
    -Power point
    -music
    -Scripts for hypnobabies

    ESLR Check

    1. I think I've been an effective learner because I don't just sit there in class and go with out my questions answered.  If I have a question I ask it, and I know that my peers may have the same questions as well. I do my best to grasp the concept and I try learning as mush as possible and sometimes my brain doesn't recognize the material and I get frustrated but I go out of my way and try learning it. Like I usually stay and talk to Mr.Ogden if I don't get the math that day, which is a normal thing for me. I usually bug him too in class when I don’t get something, and when I ask something I know I’m not just asking for me, I ask  for Jackie and J.P too at times.  As well as for the W.B I needed help and went to him and after I got help I helped others.
    2. As for my senior project I think I have been a great effective learner! I have been going more and more to service learning (which is better by the way) and no doubt about it every time I go I learn so much! I may read a lot but its not compared to the real life thing. I always have questions for my midwife, and my brain soaks them up like a sponge each time. When I read I highlight what needs to be answered and I ask it whenever I get the chance. I also bug Mrs. Ortega! haha.
    3. Some ways I can improve in this semester on the ESLR would be to ask more questions in physics, I usually get lost and leave the class with out knowing what just happened. But lately I have been bugging my peers to explain the material to me. So I can improve my ESLR in that class.

What is an effective learner at I-Poly?
Someone who takes initiative and doesn't just sit around waiting for things to get done. I feel an effective learner is somewhat independent and communicates with the teachers enough to know what is going on, what will go on and how they are doing.  Once they know all these things, they help their peers and explain it to them so we as a graduating class can be on the same page.

Service Learning 10 Hour Progress Check

    Service Learning Time log

Sep. 7: My first day of service learning was very nerve racking! I was given a tour and a run down of what would basically happen when I would go.  Very simple and easy day, a couple came in and I met them and watched a birth video after.
    1-5

Sep.8: I was asked to come in this day because a different couple had an appointment.  It was difficult interacting with the couple because it’s a very private thing to enter the room during a prenatal check up.  That’s the hardest part about my service learning, but it was nice actually seeing how it would beJ I pictured myself doing it one day!
    3: 00-5:00

Oct.26: I was watching a birth video and I was taking notes when a very nice couple came in. They had a post-natal check up and had a very cute baby boy! They told me how the mother had a water birth in the birth center, and how relaxed she was.  After them a couple cam in for a post-natal check up and from the other room I could hear the baby’s heart beat. I wasn’t sure if it was a boy or girl.
    2:00-5:00

Nov. 10: This was the best day! I had a lot of fun, and felt very comfortable with the mommy’s and the soon to be mommy’s! I also asked a lot of questions to my midwife Joyce and watched another birth video with her!
    2:00-5:00

Jan.11: This was the first time I attended service learning at night.  It was a three hour class on Hypnobabies. It was a great class! I loved it, I almost fell asleep because the class is very relaxing with soft music in the background, and they make you close your eyes and hear stories.
*Well this class opened my eyes to a different birthing technique and a different way of handling the pain.
    6: 30-9:30

Jan.12: This day at service learning was very slow, but also helpful.  I had free time to read my midwifery books and ask my midwife Joyce questions. Every time I go, I learn something new and that’s always nice! There was a pre-natal check up, and I interacted with the mother for a little bit.

    2:30-5:00

-The most significant thing I have learned from this experience is how hard it will be.  I thought it would be cherry pie and I was going just join every check up, but in reality it’s very difficult entering a private part of woman’s (which we are both strangers to each other) experience.  You have to build a special relationship with her, but not just her, with her family, her life and her baby. Which most OB’s fail to do.  From this experience I saw how it will actually be in life, and realized anything that is worth it requires hard work.
